The Madison County Health Department is committed to preserving, promoting, and protecting the public health and well-being of Madison County residents by preventing diseases, protecting the environment and promoting healthful living.

The Health Department offers information and services in Maternity Health, Environmental Health, Family Planning, Health Check, Health Choice, Bioterrorism Preparedness, Health Promotion, Health Education, Breast and Cervical Cancer, Control Program, Communicable Disease Control, Daycare Nursing, Social Work and Vital Records.

Vital Records Information is provided below: 
Birth Certificates - Assists in the preparation of birth certificates.
Death Certificates - Assists in the preparation of death certificates.
DHHS 1660 - Affidavits of Parentage for children born out of wedlock.

Appointment Procedure
No appointments necessary.  It is recommended that you call 649-3531 ext 229, prior to arrival.
 
What to Bring 
Drivers License and Social Security card.
 
Paying for Services 
No charge for Vital Records services.  However, certified copies of Birth & Death Certificates can be obtained through the Registrar of Deeds office for a fee.
